Care Plans and Health Assessments
Care Plans are available for patients with a chronic disease. These plans allow you, the patient, to learn more about your disease and assistance that you may be eligible for, eg podiatry, dietitian, physiotherapist. Medicare allows several services per year for eligible patients.
The initial assessment and paperwork is conducted by several of our nurses who have specialised knowledge in this area. You are also scheduled a consultation with your GP who will discuss the plan with you and do any necessary referrals.
